Application for a Statement in Principle
To be completed by the employer in respect of a position. Wherever possible, the form should be completed in advance of the recruitment process. Please note that when making multiple applications it is necessary to submit a separate application form for each position. The Application Form for a Statement in Principle is supported by Microsoft Word and can be downloaded and completed electronically. Payment will also need to be made.

Application for Housing Licence on Grounds of Employment - (HC5e)
To be completed by the employee once suitable accommodation has been found. This form is also used when changing address part-way through the licensed period.
Short-Term Housing Licence
To be completed by the employee, employer and the head of household/owner of the property. Payment will also need to be made.

To be completed by the employee and the head of household/owner of the property.

Non-Employment-Related Housing Licences
Application for Housing Licence Other than on the Grounds of Employment - (HC5)
To be completed by the applicant once suitable accommodation has been found. This form is also used when changing address part-way through the licensed period.
Co-Habitational Licence - (HCef)
To be completed by the applicant and the residentially qualified individual with whom the applicant wishes to cohabit. This form is also used when changing address part-way through the licensed period.
Only applicable from April-October inclusive.

Declarations of Lawful Residence
Member of the Household - (RW)
To be completed by the householder and relative as defined by The Housing (Control of Occupation) (Guernsey) Law, 1994.
Part A Open Market Private Dwelling - (RWA)
To be completed by the occupant of an Open Market Part A dwelling.
Employee in Part A Open Market Private Dwelling - (RWA Employee)
To be completed by the employee who must also be the occupant of the Part A Open Market dwelling. Payment will also need to be made.
Part B Open Market Hotel & Part C Care Home - (RWB/RWC)
To be completed by the employee who is living and working in a Part B or Part C Open Market dwelling. Payment will also need to be made.
Accommodation Owned by the States of Guernsey - (RWSG)
To be completed by the tenant and States Tenancy Department.
Part D Open Market Lodging House - (RWD)
To be completed by the owner who must also be the occupant of a Part D Open Market dwelling.
Boat Crew Declaration - (RW4)
To be completed by the applicant/crew member and employer.

Certificate of Ordinary Residence - (COR)
To be completed by an upstanding member of the community to confirm an applicant's residence in Guernsey.
Certificate of Ordinary Residence (Partner) - (COR/G)
To be completed by an upstanding member of the community to confirm an applicant's co-habitation in Guernsey.
Certificate of Ordinary Residence (Minor) - (COR/M)
To be completed by an upstanding member of the community to confirm an applicant's residence in Guernsey as a minor.

To be completed by the applicant and owner of the Open Market dwelling to be inspected. Payment will also need to be made.

Replacement Right to Work Document - (RD1)
To be completed by the applicant to replace a lost or misplaced document. Payment will also need to be made.
