Key to this role will be your excellent care skills to ensure that patients in your care have the relevant knowledge, support and resources needed to return home safely and confidently. In order to achieve this you will be fully supported to develop professionally and train to achieve your best potential. In addition you will provide training, consultation and supervision as delegated by the Senior Practitioner for less experienced social workers, support workers and student social workers.
